Commonwealth Home Support Programme

What is the Commonwealth Home Support Programme ("CHSP")

The Commonwealth Home Support Program (CHSP) is an ‘entry level’ program for people aged 65 years and over, providing basic level care needs and support for people to stay living at home longer.

Support at Home Occupational Therapy is engaged by the Department of Health to provide Occupational Therapy Services to clients that have been allocated an Occupational Therapy referral code through the Commonwealth Home Support Program

(“CHSP”).


How to access support though CHSP

My Aged Care is the single-entry point for all government subsidised aged care services. 

To be eligible for CHSP services, you must be aged:

  • 65 years or older (50 years or older for Aboriginal or Torres Strait Islander people), or;
  • 50 years or older (45 years or older for Aboriginal or Torres Strait Islander people), on a low income, homeless, or at risk of being homeless.

My Aged Care will assess which types of services you are eligible for and provide you with approvals for these services.

Once you have the approvals, you can then choose Support at Home Occupational Therapy as the provider of your services.

Your journey with us

Referral Received

Contact us with your referral code(s) from My Aged Care or ask My Aged Care to send your referral code(s) directly to us.

Intake Information and Appointment Booking

Once your referral is received, we will contact you to discuss your needs, collect intake information and arrange an appointment time. We will also discuss any fees you may need to pay during this discussion.

You are welcome to ask our staff any questions you may have regarding your services during our first discussion and at any time afterwards by contacting our office.


Initial Appointment

Our friendly and professional Occupational Therapists will attend your home to complete an assessment and determine your needs. At this appointment, we will work with you to develop a support plan which will detail your ongoing support needs.

Follow Up

If a client requires equipment or home modifications, we will make

recommendations for these and arrange ordering or completion using a clients Home Modification and Goods, Equipment and Assistive Technology referral codes.

If a client does not have the referral codes to meet their needs, we can arrange a referral to My Aged Care on your behalf to access these. 

We are here to help.


Staying In Touch

For those clients requiring follow up or ongoing supports, we will assist with this.

We can also provide review visits down the track as a client's needs change or further issues arise.

In addition, we may be able to support you if you are assessed to access other services through My Aged Care in the future.
