Our services are available through the following funding pathways:


  • Support at Home (via an approved provider)
     
  • Restorative Care (via an approved provider)
     
  • End of Life Pathway (via an approved provider)
     
  • Commonwealth Home Support Program (CHSP)
     
  • Privately Funded (a Medicare rebate may be available)
     

If your funding type isn’t listed above, please don’t hesitate to contact us - we’re happy to discuss your circumstances and advise whether we’re able to provide support.


Work With Us

1. Complete Referral Form

Complete our Referral Form and return it to us with any relevant accompanying documents.

2. Receive Booking Confirmation

Within 1-3 business days, you will receive a booking confirmation email outlining details of the booking and the quoted charge based on the information provided within your Referral Form.


If we are unable to contact a client to book a service, we will provide you with regular updates as to our attempts at contact.

3. Receive Report

Within 3-5 business days of service provision, you will receive a comprehensive report outlining the assessment findings and recommendations. 


Diagrams and/or photographs will be included for any recommended home modifications. 


Quotes for any recommended equipment and/or home modifications will follow as soon as received by us.

4. Provide Approval

If a client requires a trial for recommended equipment items (or any other necessary follow-up action), a quote for the additional service will be provided and approval will be sought from you before we proceed. 


Following receipt of approval, equipment trials will typically be scheduled within 7-10 business days (subject to supplier availability). 


Joint site visits for home modifications may fall outside this time frame. 


A supplementary report and any associated quotes will be provided to you within 3-5 business days. 

5. Receive Review Reminder

In line with the review date recommended by the OT within their report, you will receive a follow-up email reminding you that the client is due for OT review. 


Review is typically recommended 12 months post assessment, or sooner subject to functional decline.

INFORMATION FOR BUSINESSES

  • If you are an approved provider that is referring to us for the first time, you may require a service agreement completed.
    We are happy to arrange execution on request.
  • We are able to provide compliance documentation on request and are able to be setup on compliance portals.
  • We enjoy the opportunity to work with self-managed home care clients and will complete the required documentation and onboarding to enable this.
  • We offer transparent pricing and quoting to make it easier to work within client's budgets. We are also happy to work with purchase order systems.
  • If you or your client have preferred equipment and home modification suppliers, then please let us know and we will endeavor to use these suppliers in the first instance.
